VA Disability Claim Evidence: Build an Organized Record

Evidence should answer the actual questions raised by the claim. A large file is not automatically a strong file; organization helps show what each document establishes and where information may still be missing.

Core records

VA identifies records such as separation documents, service treatment records, and medical evidence related to the claimed illness or injury as common evidence sources.

Lay evidence

VA also accepts lay evidence, including statements from the Veteran or others with personal knowledge of relevant events, symptoms, or changes.

Label evidence by purpose

Instead of storing documents as one undifferentiated stack, note the condition, date range, source, and what the document is intended to show. This makes later review faster and clearer.
